Important Information

This tool provides a general framework for person-centred support planning. It should be adapted to the person, the setting and the type of support being provided.

Services should also make sure they meet any legal, professional, regulatory and organisational requirements that apply. Extra records or assessments may sometimes be needed.

If there are concerns about health, safety, capacity, safeguarding or legal rights, the correct professional guidance and procedures should be followed.

The person comes first. The paperwork comes second.
